SUBJECT: Revised Affidavit for Arizona Corporation Commission

The Arizona Corporation Commission recently revised its Affidavit of Publication form newspapers use concerning publication of corporate Public Notices.

These would include:

Articles of Incorporation, Articles of Organization, Articles of Domestication, Articles of Amendment, Articles of Restatement, Articles of Merger or Share Exchange, Articles of Dissolution, an application for Authority to Transact Business and an Application for Withdrawal.

But perhaps more importantly, you no longer should attach a copy/clip of the notice you print.

Only complete the affidavit. Do not attach a copy of the published notice.

For other public notices where a copy/clip is needed, please continue to use your current form. But for Corporate public notices, please use the revised Affidavit.
